Complete Physical
Many Doctor’s offices have the physical forms available in their office, but you can also download the form here. Your athlete will not be able to participate until they have completed a physical.
Purchase Game Day Meals
Every athlete will need to purchase Game Day Meals for the season. Meals will be provided to athletes before each game (home and away). If you require financial assistance for Game Day meals, please contact cpmsathleticsinfo@gmail.com.

Volunteer
Complete Rank1 Forms
Ensure you complete your Rank1 forms. Your athlete will not be able to participate until they have completed their Rank1 forms.
Become an Approved LISD Volunteer
All volunteers MUST be approved by the Leander School District. Filling out the form takes minutes. Please allow 2 days for approval to go through.
Make sure you plan on signing up for at least ONE volunteer position during the season! Parent volunteers will be NEEDED throughout the season.
